La CAP Agencies
This will bring you to the Louisiana map that is color coded by regions A - H.
When you click on your parish's map area or the letter associated with your
parish region, you will be given agency contact information for your parish.
Contact
This page can be used to send information for contact or response from CAPC
or click on the email links at the bottom of the form to send directly to the
Program Director or Coordinator for contact or response.
Client Input
You can use this electronic form to send your client information
to the CAPC office or you can fax your CAPC Client Referral Sheet
to 225-298-3325 (assistance needed).
Client Update
Your Client Input information can be updated here (services provided).
