COMMUNITY ASSISTANCE PROGRAMS

ALL APPLICANTS MUST MEET THE GRANT GUIDELINES TO QUALIFY FOR ASSISTANCE.

THIS IS CSBG FUNDED PROGRAM FOR ASSISTANCE

Lafourche Parish Council Office of Community Action is an equal opportunity provider.  We consider applications for assistance from all residents without regard to Race, Color, Religion, Sex, national origin, Age, disability, marital or veteran status, or any legally protected status.

Lafourche Parish Council Office of Community Action acting as Sub-Grantee of the Community Service Block Grant funding program is accepting applications to assist Low- income, under-employed, un-employed, or residents.

  1. RENTAL/MORTGAGE ASSISTANCE– This Program is to assist Low-Income residents of Lafourche Parish with Rental/Mortgage assistance to prevent eviction and or Bank/Mortgage foreclosure on their existing homes. Qualified applications will be accepted and will receive services for a maximum of one (1) month, not to exceed ($1,000.00) per qualified applicant.
    • Applicant must provide Letter from Landlord/Mortgage Company/Bank approving acceptance of payment of qualified amount for delinquent rent/mortgage.Letter must specify the amount of delinquency and must accompany a completed and signed W-9 form from the Landlord/Mortgage Company/Bank.
  2. UTILITY ASSISTANCE– This Program is to assist Low-Income residents of Lafourche Parish with their emergency utility needs to prevent disconnection of utility services on their existing homes.Qualified applications will be accepted and will receive services for a maximum of one (1) month, not to exceed ($450.00) per qualified applicant.
    • Applicant must provide Copy of Electric, Gas, Water Utility bill in applicants name for current month showing delinquency and disconnect date.
  3. MEDICINE/MEDICAL ASSISTANCE– This Program is to assist Low-Income residents of Lafourche Parish with their emergency medical and medicine prescription needs to prevent qualified applicants from receiving necessary medical treatment and required medical prescriptions due to lack of income.Qualified applications will be accepted and will receive services for a maximum not to exceed ($200.00) per qualified applicant.
    • Applicant must provide Copy of note from attending physician stating that the services provided and/or medication prescribed is medically necessary for the required treatment.
  4. FOOD ASSISTANCE– This Program is to assist Low-Income residents of Lafourche Parish with their emergency Food needs to prevent lack of nutrition and or food products in their existing homes.Qualified applications will be accepted and will receive services for a maximum not to exceed ($100.00) per qualified applicant.
    • Applicant must provideProof of income of ALL members of the household.A list of nutritional Food items will be provided to each qualified applicant receiving assistance
  5. EDUCATION/TUITION – This Program is to assist Higher Education students enrolled in an accredited College or Technical College with Tuition and or books to subsidize the rising cost of Higher Education.Qualified applications will be accepted and will receive services for a maximum not to exceed ($500.00) per qualified applicant.
    • Applicant must provide Proof from Accredited University/Technical College showing enrollment status for students in Higher Education requesting tuition/books assistance.
  6. EDUCATION/SCHOOL UNIFORMS- This Program is to assist students enrolled in the Lafourche Parish School System pre-k through 12, with mandated school uniforms. This Program is also to assist students attending a secondary educational program that required uniforms.Qualified application will be accepted and will receive services for a maximum not to exceed $50.00 per qualified applicant.
  • Applicant must provide Proof from Accredited Technical College showing enrollment status for students in Higher Education requesting school uniform assistance

ALL APPLICANTS MUST ALSO PROVIDE THE FOLLOWING DOCUMENTATION WHEN COMPLETING AN APPLICATION TO DETERMINE ELIGIBILITY FOR ASSISTANCE:

  1. Drivers License or Picture I.D. and Social Security Card(s) of everyone in household
  2. Proof of Income (Last 30 days of income: 2-4 Paycheck stubs) of ALL members of household.Students requesting Tuition/Books assistance Proof of income of the student, if living in own housing, and Proof of Parent/Guardian income if student is living at home with Parents (also requires current and previous months check stubs).
  3. Proof of current residency (Copy of utility bill in applicants name and/or proof of residency)
  4. All household bills from the last 30 days for rental or utilities assistance (Utilities, insurance, house or rental, phone, cable, credit card, medical, etc)
